INDIANA MEDICAID WAIVER SERVICES

Life Strategies provides person-centered Indiana Medicaid Waiver services that help individuals with intellectual and developmental disabilities build greater independence, achieve meaningful life goals, and actively participate in their homes and communities.

Life Strategies provides services through Indiana’s Medicaid Home and Community-Based Services (HCBS) Waiver Program, helping individuals with intellectual and developmental disabilities build independence and achieve meaningful outcomes.

Individuals receiving the Family Supports (FS) or Community Integration and Habilitation (CIH) Waiver may be eligible to receive services from approved providers like Life Strategies. We proudly serve individuals and families throughout Lake, Porter, St. Joseph, Elkhart, Marshall, and Kosciusko Counties.

Our experienced Behavior Consultants provide Behavioral Support Services (BSS) using individualized, evidence-based supports that promote independence, community participation, and an improved quality of life. Every service is tailored to each individual’s strengths, preferences, goals, and support needs.

Services may include Functional Behavior Assessments (FBAs), Behavior Support Plans (BSPs), consultation, caregiver and staff training, and ongoing monitoring to help individuals achieve meaningful outcomes across home, work, school, and community settings.

We partner closely with individuals, families, guardians, Support Coordinators, and each individual’s Individualized Support Team (IST) to provide practical, person-centered supports that foster lasting success and meaningful outcomes.

Your Behavior Consultant provides individualized, person-centered supports that may include:

  • Functional Behavior Assessments (FBAs) to better understand support needs
  • Development of an individualized Behavior Support Plan (BSP)
  • Coaching and training for families, caregivers, and support staff
  • Ongoing consultation, data review, and plan adjustments
  • Services provided in the home, workplace, day program, community, or other appropriate settings
  • Collaboration with the individual’s Support Coordinator and Individualized Support Team (IST) to promote meaningful outcomes

Behavioral Support Services (BSS) are individualized to each person’s strengths, goals, and support needs. Services may help with:

  • Safety awareness and community safety
  • Social and relationship skills
  • Communication and emotional regulation
  • Aggression or self-injurious behavior
  • Property destruction
  • Daily living skills, hygiene, and toileting
  • Workplace and vocational success
  • Healthy routines and transitions
  • Food-related challenges
  • Building greater independence at home and in the community

Your Behavior Consultant provides individualized, person-centered supports that may include:

  • Functional Behavior Assessments (FBAs) to better understand support needs
  • Development of an individualized Behavior Support Plan (BSP)
  • Coaching and training for families, caregivers, and support staff

Life Strategies provides Psychological Therapy through Indiana’s Family Supports (FS) and Community Integration and Habilitation (CIH) Waivers. Our licensed clinicians provide individualized, evidence-based therapy that supports emotional wellness and improved quality of life.

Psychological Therapy may help with:

  • Anxiety and depression
  • Trauma and grief
  • Emotional regulation
  • Stress and life transitions
  • Family and relationship concerns
  • Substance use concerns
  • Healthy coping skills

Services may include:

  • Individual, family, and group counseling
  • Evidence-based therapeutic approaches, including Cognitive Behavioral Therapy (CBT)
  • Crisis intervention and emotional support
  • Collaboration with the individual’s Support Coordinator and Individualized Support Team (IST)

The Family Supports (FS) Waiver provides non-residential services that help individuals live more independently while remaining active in their homes and communities.

The Community Integration and Habilitation (CIH) Waiver provides comprehensive supports for individuals with intellectual and developmental disabilities who require a higher level of ongoing assistance. The waiver helps individuals live successfully in their homes and communities while promoting independence, self-determination, and meaningful community participation.

Like the Family Supports (FS) Waiver, services are individualized and based on each person’s assessed needs, goals, and support plan.

HOW DO I APPLY FOR INDIANA MEDICAID WAIVER SERVICES?

Individuals can begin the application process as soon as they have been determined eligible for Bureau of Disabilities Services (BDS) programs. Applying early is encouraged, as some services may have waiting lists.
